Abstract
Study Design: Review article.

Objectives: To provide an update on the pathophysiology and neuroprotective treatment and the timing of surgical treatment after acute spinal cord injury.

Summary of Literature Review: A literature review was conducted with a particular focus on the development of pathophysiology and emerging neuroprotective treatment of acute spinal cord injury, as well as the effectiveness of early decompression.

Materials and Methods: Review of the relevant articles.

Results: After a primary mechanical impact, a complex cascade of secondary injuries follows in an acute spinal cord injury. Neuroprotection is the main strategy to treat acute spinal cord injury. Beyond methylprednisolone, several other pharmacological treatments and methods have been studied for clinical research. Animal studies support early decompression of an injured cord. Although there is no consensus regarding the timing of decompression, early decompression in humans has many advantages.

Conclusions: There is no standard treatment for acute spinal cord injury. Although still debatable, high-dose steroid treatment is one option for spinal cord injury. Further clinical studies are on the way.
